When you schedule your initial consultation with Southwest Orofacial Group, we'll send you a link to establish your Patient Portal account. 

One week before your first appointment, you'll receive an email indicating that new forms have been added to your Patient Portal. These forms include your health history, information about your symptoms, and treatment consent forms. Your doctor must review all of this information before your visit can begin, so it's important that you complete your registration PRIOR to your appointment date. 

After your initial consultation is complete, the Patient Portal is where you can review your Visit Summaries, pay your balance, request prescription refills, or send us questions.
